Preparing for
your first visit:
Welcome to our Practice. Prior to beginning treatment at a Premier Medical Group office, we will need your general history information. You may complete this information at your first visit, or at your convenience ahead of time, which we recommend.
In the section below, you will find printable General Forms, Accident/No Fault/Injury and Assignment of Benefits Forms that you may print at home, complete and bring with you to your first visit. Please complete the
General Health Information , Workers Comp/ No Fault/ Medi-Pay Insurance and Assignment of Benefits forms in their entirety.
Be sure to bring your insurance card with you and have it available for the receptionist upon checking in. In addition, please obtain your referral authorization from your primary care or specialist physician prior to your first appointment.
To help ensure a productive first visit, bring all previous X-Ray/MRI studies and reports, along with all medical records (office notes, surgery reports, etc.) pertaining to your injury or condition(s) with you to your first appointment.
We ask that you notify us at least 24 hours in advance if you cannot keep your appointment. For further details regarding our “Office Policies”, click here.
